We are in the process of revising our critical care attestation. Is anyone willing to share their critical care attestation/statement with us? Do you require providers to document start and stop times?

Hello, I am willing to share what we have.  For adult critical care, providers use departmental templates, so they don't really use a standard critical care attestation as things are built into the template. However we did create our adult critical care users a Critical Care Split Shared attestation, Critical Care during global surgery, and Critical Care modifier 25 attestation for all the "special scenarios" that arise. I don't think they get high volume usage yet, but they are available. Last year we created a net new neonatal critical care attestation, so it doesn't include time but it has some helpful language in my opinion.

We do not require start and stop times, especially considering critical care can be intermittent throughout the date of service. We do require at minimum a "total critical care time" spent statement. My email is mlondon5@bsd.uchicago.edu, feel free to reach out for the statements and such!
