  • 1.  Mult-D visits

    Hello.  Looking to see how others handle the facility coding for a multi-d visit where the patient remains in one room but has separate and distinct visits from various providers.  Could be 2 MDs from different disciplines and could also include a visit from a dietician or physical therapist. Some suggestions are to use G0175, but that is a scheduled team conference and our interpretation is that code is for everyone meeting at the same time with the patient.  In our scenario, the visits are consecutive.  Another suggestion would be to use G0463 for each separate MD visit with a G0(zero) condition code on one of them to show multiple visits in the same revenue center. And lastly, some think only one G0463 is billed since the patient does not travel to the other clinics but remains in a single room. This does not seem to capture the resources (extended time in clinic room). Please let me know how your organization handles multi-D visits.  Thank you so much!!  Mary

    Mary, we bill only one G0463 for Multi-disciplinary clinics as the patient is only in one room, checked-in to one room, and all the facility resources and RN/MA staff that contribute to that visit are factored into the facility level (i.e. our internal facility level charge might be a 99214 which is updated to G0463 on the Medicare claim). Then each specialist that sees the patient bills professionally with their own note that includes their distinct evaluation and management of the problems related to their scope.  So multiple professional fees, and only one facility E/M.
