  • 1.  SSN Disclosure Statement

    For those of you who are government or state agencies, do you include the SSN disclosure statement in any forms when you collect SSN from patients?

    Upon requesting a SSN, the Federal Privacy Act of 1974 requires all government agencies (including state agencies) to provide a disclosure statement with the following information:
    • whether disclosure of the SSN is voluntary or mandatory;
    • the authority or legal statute requiring mandatory disclosure of the SSN;
    • how the SSN will be used; and
    • the consequences, if any, for failure to provide the SSN.

    We are a state agency and we collect SSN for different purposes (Medicaid, Workers Comp, etc.). We are working to include the required disclosure statement in our consent forms. But we want to check with you all here to see how you have managed this requirement.
